  • Proof of MC Authority

  • Taxpayer ID #/EIN

  • Proof of Cargo Insurance

  • Remit To/Factoring Company Mailing Address & Contact#

Additional Documentation Appreciated (but not required):

  • Copy of Certifications - Minority/Women-Owned, SmartWay, SCAC Certificate, ISO Certified, etc

  • Equipment Types - # of power units/trailers, service area, specific lanes of interest, etc
